Between 10% and 19% of patients discharged after a heart failure decompensation are readmitted within 30 days.
To reduce this percentage and prevent decompensations in the month following discharge, the innovation units and the Internal Medicine and Cardiology departments at Hospital Son Espases, together with AstraZeneca's innovation area, launched a project where artificial intelligence (AI) plays a key role, powered by Tucuvi's technology.
LOLA, Tucuvi's clinical virtual assistant, enables healthcare professionals to carry out personalized, continuous follow-ups for these patients from their homes — without the need to travel to a healthcare center. LOLA calls patients daily, asking about signs of decompensation. The platform automatically transforms patient responses into structured information using AI and generates alerts to identify those at higher risk of decompensation or those with changes in their usual symptoms — allowing clinical teams to focus their time on patients who need intervention.
This data is combined with the work of healthcare professionals.
First, nurses review the information from patients with warning signs, and if early signs of decompensation are detected, doctors take the necessary decisions to prevent disease progression and avoid unnecessary hospital readmissions.
The results from the first six months of follow-up (June to December 2024) show a significant impact: only 1.6% of heart failure patients were readmitted due to decompensation, while the national average is much higher, exceeding 10%.
Additionally, the system has been widely approved by users, who rated it with a satisfaction score of 4.7 out of 5.
With this innovative project, Hospital Son Espases, in collaboration with AstraZeneca and Tucuvi, positions itself at the forefront of healthcare — using artificial intelligence to improve the efficiency and effectiveness of monitoring complex conditions such as heart failure.
